OFFICE POLICY

Fees and Payment:

Payment is necessary prior to or at the time of services, depending upon payment method. Payment may be made by check, cash, money order or credit card. There will be a $25 charge for any check returned by the bank. If not using a credit card for telephone or online services, a check or money order must be received prior to the scheduled session. (note: secured credit card transactions online will be available in February)

My professional fees are based on $150 for a standard in office 50 minute session, unless otherwise negotiated. Online (telephone, video, chat and e-mail) services are billed at $125 per hour. Brief professional services are billed at $30 per 15 minutes or any part there of, including phone consultations. While fees for services may vary according to the particular service provided, they generally range between 2 to 3 dollars per minute. Details can be discussed upon request. Click here to review special offers.

Note: Cyber-Psychologist Consulting does not provide insurance billing for these services. However, upon your request, we will provide you with a bill that you can submit to your insurance company. It is unknown whether your insurance company will reimburse you for these services.

Scheduling:

Since effective treatment often involves consistent, ongoing sessions, it is recommended that we schedule a time that we could maintain regularly. If a situation arises and you must cancel any appointment with less than 24 hour notice, you will be held financially responsible for the session. (Exceptions to this policy may include emergencies such as illness or accidents). You may contact Dr. Silva for an appointment by telephone, E-mail or instant messaging online. She may be reached at (808) 651-1453, or toll free at (800) 484-2680, pin# 2680. You may also schedule now by submitting an instant message.

PRIVACY POLICY

Confidentiality and Security:

All services provided by Dr. Silva and Cyber-Psychologist Consulting are strictly confidential. No information may be released to any third party without specific consent, usually in writing. We are committed to respecting your privacy within the bounds of the law.

Exceptions to this are in cases of abuse and in cases where you indicate you might hurt yourself or another, or cause damage to their property. In such cases of suicidal intent, homicidal intent, or abuse of another, healthcare providers are required by law to report the information to the appropriate authorities.

Cyber-Psychologist Consulting recognizes the sensitive nature of services to its clients. Under no condition will we ever share or sell any information about you with any other individual, web site, or company for any reason. Dr. Silva and Cyber-Psychologist Consulting are committed to respecting your privacy and will safeguard your privacy in connection with using our website and speaking with a consultant. We operate under the following set of strict privacy principals:

We provide an online privacy policy statement to inform the client.

All personally identifiable information such as name, E-mail address, mailing address, phone numbers, credit card information, driver’s license number, etc, will not be disclosed to anyone unless the client indicates we may do so.

Cyber-Psychologist Consulting employs strict security measures to safeguard all client records and online transactions. Personal information necessary for provision of services and processing of payments is always processed and stored in a private and secure manner.

LIMITATION OF LIABILITY

The client agrees that use of any and all services provided by Cyber-Psychologist Consulting and Dr. Silva is entirely at the client’s own risk. Services are provided “as is”, without warranty of any kind, either express or implied. Specifically, any and all warranties are disclaimed, including without limitation any warranties concerning the availability, quality, accuracy or content of information, products or treatment services or outcome. This disclaimer of liability applies to any damages or injury caused by the failure of performance, error, omission, interruption, deletion, defect, delay in operation of transmission, computer virus or breakdown, communication line failure, theft, destruction or unauthorized access to, alteration of or use of record, whether for breach of contract, tortious behavior, negligence or under any other cause of action. Neither the provider nor any of its agents, affiliates, or content providers shall be liable for any direct, indirect, special or consequential damages arising out of the use of the service or inability to gain access to or use the service, or out of any breach of any warranty. The client hereby acknowledges that the provisions of this section shall apply to all content on these services.
